What are the documents required for insurance claims?
For cargo claims
Documents generally required for settlement of various types of cargo claims are as under. Keeping in view the purpose of calling for the claim documents and depending on circumstances, submission of certain documents may be waived recording the reasons for the same:

  • Original insurance policy/declaration under the open policy duly endorsed by the assured/claimant.
  • Original or signed copy of sales invoice along with packing list.
  • Copy of bill of lading (in case of ocean transit).
  • Copy of bill of entry.
  • In case of general average,counter-guarantee or original cash deposit receipt with letter of transfer
  • Letter of Subrogation duly stamped (only where recovery from carriers in possible) Special Power Of Attorney, whenever applicable.
  • Lost Overboard Certificate where loss has taken place during loading or discharge (sling loss)
  • In case of short landing or non delivery of complete consignment: original contract of afferightment duly endorsed, viz: Railway Receipt, Transport Receipt, Full Set Of Bill Of lading, Air Consignment Note, Postal Receipt, etc, as applicable. The original contract of affright should be endorsed by the carrier confirming shortlanding / non-delivery of the entire consignment or a separate shortlanding /non-delivery certificate.
In case of partial loss/damage
  • Open assessment report by the carrier, and/or.
  • Survey report of independent survey.
  • Claim form and claim bill.
  • Copies of correspondence exchanged with carriers, whenever recovery from carriers is possible
  • Bankers Certificate confirming non receipt of export proceeds in India in an approved manner (incase of claims under export policies settled in India).
What are the documents required for insurance claims?
Claims due to fire and / or Explosion
  • Copy of Claim intimation given to Company together with xerox of policy & premium receipt
  • Duly filled Claim Form
  • Policy Panchnama/F.I.R./Final Police Report (Forensic Deptt)
  • Fire Brigade Report
  • Photographs of Damaged Property showing extend of damage & / or video film of loss
  • To substantiate quantum of loss, value of damaged insured property prior to loss & value of salvage. Please make available to surveyors./investigators fixed fixed Asset Register, original Bills/Invoice
  • Repairs/Replacement bills/Invoices and stock register
Claim due to Burglary / Theft of Vehicle
  • Copy of Claim intimation given to Company together with policy & premium receipt
  • Duly filled Claim Form
  • Policy Panchnama/First Information Report/Final Police Report (Forensic Deptt) Policy investigation report
  • Photographs of Damaged Property showing damage & / or video film of loss
  • To substantiate quantum of loss, Please make available to surveyors /investigator , your stock register, original Bills/Invoice, cash memos, fixed asset register, bank statement, cashbook/ledger
  • Inventory of articles kept in bank-locker
  • Driving Licence (DL) Original with xerox copy
  • Registration Certificate of Vehicle (RC) (Original with xerox copy)
